A função de publicização do acompanhamento terapêutico na clínica
o contexto, o texto e o foratexto do AT
Resumo
Esta pesquisa investiga o Acompanhamento Terapêutico (AT) como dispositivo clínico-político e suas implicações na atenção à saúde mental no contexto da Reforma Psiquiátrica no Brasil. Construímos o trabalho em três momentos: num primeiro traçamos o contexto em que tem se dado as experiências de AT; num segundo trouxemos à cena os textos sobre AT e, num terceiro, incluímos o foratexto, ou seja, problematizamos e fizemos interceder no texto e no contexto as funções que tal dispositivo faz funcionar. O AT surgiu num movimento de desinstitucionalização da loucura, tomando a cidade como campo de experimentação e inserindo-se para além dos estabelecimentos de saúde. Realiza uma “clínica sem muros”, problematizando a um só tempo a doença mental e sua relação com os espaços urbanos e interrogando radicalmente as práticas manicomiais. Paradoxalmente, há uma tendência no campo do AT de institucionalização e privatização da clínica, fazendo do mesmo um especialismo. Acompanhando o dispositivo em suas variações e em sua articulação com a rede de saúde mental brasileira, atentamos aos agenciamentos que estabelece com a Psiquiatria, com a Psicanálise, com as Universidades e com a cidade e os efeitos políticos que tais agenciamentos produzem. As relações que se estabelecem entre os vetores que compõem o plano de experimentação clínica do AT, dão visibilidade a algumas funções que o dispositivo AT opera: função micropolítica, função de transversalização, função rizomática, função deslocalizadora e analisadora da clínica, função de resistência aos modelos centrípetos e analisadora do Movimento da Reforma Psiquiátrica brasileira, função de territorialização, função inclusiva e função de publicização, ou de geração de um plano comum na clínica. O AT comparece como um mobilizador de forças capazes de consolidar um estatuto público para a clínica.

Abstract
This research investigates the Therapeutic Accompaniment (AT) as a clinical-political device and its implications in the mental health attention in the context of the Psychiatric Reform in Brazil. We constructed the dissertation in three moments: in the first one, we outlined the context in which experiences of AT have been happening; in a second moment, we brought to the scene the texts on AT, and in a third moment, we included the out-text, that is, we problematized and made intercede in the text and in the context the functions that such a device sets at work. AT arose during a movement of deinstitutionalization of madness, taking the city as its field of experimentation and inserting itself beyond the very health establishments. It does a “clinic without walls”, problematizing at the same time the mental disease and its relations with urban spaces and radically interrogating manicomial practices. Paradoxally, there is a tendency in the field of AT to institutionalize and privatize the clinic, making of it a specialism. Accompanying the device in its variations and in its articulation with the network of brazilian mental health, we took into consideration the agenciations that it establishes with Psychiatry, with Psychoanalysis, with the Universities and with the city and the political effects that such agenciations produce. The relations that are established between the vectors that compose the plan of the clinical experimentation of AT give visibility to some functions that the AT device operates: micropolitical function, transversalization function, rhizomatic function, delocalizer and analyzer function of the clinic, function of resistance to the centripetal models and analyzer of the Brazilian Psychiatric Reform Movement, function of territorialization, inclusive function and function of publicization, or of generation of a common plan in the clinic. AT affirms itself as a mobilizer of forces capable of consolidating a public status for the clinic.
